Chris:

If you do not have a serious illness, expensive prescriptions, or long term injuries in your MIB (Medical Information Bureau) report, you can qualify for a much cheaper private health insurance plan

3 months only? Was that mini COBRA, or what happened? COBRA traditionally lasts about 18 months.

Certificate of Prior Creditable Coverage

Start writing to the former plan's administrator and health insurer to get a certificate for your records. Other ways to show you had creditable coverage:

Pay stubs that reflect a deduction for health coverage premiums;

Canceled checks

Ledgers or journals detailing payments

Copies of premium payments or other documents showing evidence of coverage;

Explanation of benefit forms; and

Verification by a doctor or your former health plan.

You may also be asked to swear to the period of creditable coverage and cooperate with the new plan’s reasonable efforts to verify you had creditable coverage.
