Chris:

Contact your health plan adminstrator or former insurance company. If they are both unresponsive, you might be able to use statments, paystubs, and endorsed checks to show you had the health insurance. I wrote an article about Certificates of Creditable Coverage.

The Certificate of Creditable Coverage provides once you have been continuously insured for at least 18 months there is no need to satisfy another waiting period when you change health insurance. You said you only had coverage for 3 months. 18 months is the maximum length of a waiting period but many group health polices have a more liberal waiting period of only 6 months. So you may still have to wait between 3-15 months.
