I need to verify that my dental coverage was in force last 01/14/11. My dentist submitted a claim that was denied and it should not have been. My social is XXX-XX-XXXX. please feel to call me at 314-XXX-XXXX

the have statements, or pay stubs showing you pay for the dental insurance? And you have a summary of benefits. Contact your HR department in a summary of benefits for your dental, and e-mail or fax that to be the dentist. If they deny benefits they should do so in writing and give you a right to appeal.
