Contact your health plan admin and let them know about your divorce asap. They will send your spouse the paperwork.

An initial notice has 2 be given to covered employees and spouses when the plan starts. The notice must inform them of rights under COBRA. The summary plan description or SPD must contain COBRA information.

When the plan administrator is notified a qualifying event occured, they must notice each qualified beneficiary of the right to choose continuation coverage.

COBRA gives you 60 days from the date the election notice is provided to inform the plan administrator that the qualified beneficiary wants to elect continuation coverage.
