I am currently getting COBRA insurance because I was laid off from
Med Solutions at the end of March 2010. I recently divorced and my last
name has changed.

What paperwork do I need to fill out to update this information on my COBRA account?

Also, how much longer am I elligible to receive COBRA?

It might take 30-60 days for the health insurance company to process your name change, they don't do anything fast (besides take our money).

It's the responsibility of the employee or a family member to inform the Plan
Administrator, which is also the above employer, of a divorce , legal separation, or a child losing dependent status within 60 days of the event.

Failure to notify the employer in 60 days will end your rights to COBRA continuation.

The employer only has to report the employees death, termination, reduction in hours of employment or entitlement to Medicare.
