You already had group health plan and:
Your group had over 20 total employees
Were laid off or quit
Divorced or legally separated
Are an employees spouses or dependent child
Still employed but with reduced hours
Paying the entire premium yourself
Paying up to a 2% administrative fee
Were a state or local government worker of iniquity
Are a spouse or dependent
Reduced part time hours
