COBRA questions (with answers) I have received via Email

My Employer gave me a form to fill out for cobra but there is no address where to mail the check.

The check should go to the health plan administrator. Some employers administer plans themselves. Check with your former employer, because you have a limited amount of time to get it in. We only help consumers with their COBRA problems, we don't administer plans for employers.
