You need to satisfy the Department of Labor's regulations for the content of the COBRA election notice. The Department has published a model notice in Word format on its website (dol.gov) that is a "safe harbor," meaning the Department believes that the model satisfies its regulations. You must carefully adapt that notice for your plan, as you don't want to make the mistake of offering coverage that's unitended. If you are not schooled in details of COBRA, think seriously of consulting a benefits professional before you draft and provide an election notice.
