Gary, COBRA has very strict rules. Timely payment of premiums is one. They will drop you like a hot potato if late.

Cobra law states that you have 60 days to elect coverage and 45 days to pay. Yes they are due on receipt.

As far as question 2 is concerned, If you did not make payment within the time period you may not be able to get back into COBRA

I would contact department of Labor.
