Your coverage can be terminated for nonpyament after the grace period ends or when it overlaps the new due date for payment.
Did you request your address changes in writing? If you can back up what transpired in writing and show that there was a lack of communication between the insurance carrier and employer, you have a good case.

: My COBRA coverage from previous employment was being paid by my current employer. During the 8 month period that I had coverage, I had three different premium amounts (gradually increasing). I was never notified of these changes in the premium amount, nor was I notified when the third party biller changed to another company. Although I requested that my address on record be corrected five times during the 8-month period, so that I would be aware of the amount of premium due, I did not receive a bill and was instead forced to call every month in order to find out the current premium amount due.
: My coverage was for the period of 07/01/2004 to 02/28/2005. My employer mailed a check for the premium due in February 2004, but did not send a payment for March 2004 because they were unsure of the premium amount, and I was at home on mandatory bed rest due to pregnancy. My coverage went into the grace period on 03/01/2005. I gave birth on 03/27/2005 and was released from the hospital on 03/31/2005 after a C-section. My employer contacted the third party billing company on 04/04/2005 to let them know that we still had not received a bill, so they did not know how much to pay. The customer service rep told my employer that the coverage had not been cancelled and that if the overdue premium was mailed that day, the coverage would not be cancelled. My employer mailed a check for the full amount past due plus the current month's premium that day, but when I went to get a prescription filled on 04/16/2005, I was told that my coverage was terminated on 02/28/2005. When I called to find out what was going on, the customer service rep told me that there was NO RECORD of the phone call on 04/04/2005 and that the check was returned. Well, the mailing date on the returned check was the day after I called to find out why my coverage was cancelled.

: Can coverage be cancelled if the insured is in the hospital when the 30 day grace period ends, since obviously they would be unable to make a premium payment?
