I agree with Agent about the cancellation. There is no special regulation that protects you because you were in the hospital. Also COBRA admins are not required to send invoices, so your payments should be sent on a timely basis whether you get something or not. Just pay the amount you know needs to be paid.
But I am concerned about your statement of the premiums going up three times. To my understanding, COBRA premiums must be set for a 12 month period at a time and generally cannot be changed except that once a year. This, along with the other things you stated, make me think that perhaps you could benefit by speaking with a COBRA experienced attorney.
