Yes, termination refers to both resignations and discharges. Pretty much any reason the employee stops working, with the exception of gross misconduct.
If you are enrolled in a region specific HMO and move out of the area, then IF the plan offers another non-region specific option, you should be allowed to change to the other plan. If there is no other plan, then they don't, and you are just kinda out of luck. They don't have to create a special plan.
