For Cobra purposes how are the Cobra rates established for voluntary (employee pay all) self funded or direct reimbursment dental plans?
Self funded medical cobra rates I believe have to be established actuarily. This quesion also applies even if employer is funding ee only dental coverage. This is governmental entity.
