When does coverage become effective?

Job and coverage ended on 10/31. Just received certified letter from Cobra today 11/19. If I mail this tomorrow, and I have a procedure (out patient) scheduled for Tuesday (Nov 23) - will I be covered? I was previously pre-authorized for this procedure but if Dr calls to get approval again - is this going to get denied? Doctors office doesn't have a clue about Cobra law and wants only to know where to send bill. Is it customary that proof of my cobra application be sufficient? I really needed this procedure and am a lot of pain. Thank you for the help.
