i recently voluntarily ended employment and am at a new job that i have to wait 3 months to get their insurance. in the mean time i was wanting to looking into COBRA at my previous job, the insurance Rep. says that i need to fill out a continuation form and mail it to my old employer with a check in the amount of my insurance. my old employer is too "busy" to fill out the form, sign it and send it to me. the rep. says that they have done their obligation and informed me about COBRA, which i was the one who had to keep asking to get any answers, no one told me, and that they don't have to mail me anything. I live in North Carolina and am very frustrated with why they can not send the form. i want my old employer to sign this to make it legal because i am worried that they will cancel my insurance and still take the money. what is the best way to get this form mailed to me?
are they allowed to just not send it and expect me to fill out a form and mail it with a check even though they don't know what is going on because they are too busy to find out? i have looked on this website and i see that they are supposed to mail me this form.
any kind of answer to this will help since i am without insurance now and don't know what i need to do.
