You have to contact you cobra plan adminsitrator for that. You need to get replacement paperwork ASAP before the 60 day enrollment period is over!

: I recently started a new job and can not get insurance through them till after a 6 month probation period. At my last job I was covered under Oxford. My last place of employment filled me in on Cobra. I lost the paperwork and would like the extended coverage. Could you please send me the information on who to make my payments to.
