You have 45 days from the date notification of COBRA rights to elect COBRA. You then have 30 days after the election date to pay the first premium. There is no acceptance; it is guaranteed if you elect. If you have a claim, it would not be honored until you elect and pay, but coverage will be retroactive to (and you will pay the insurance premium for) the date of job termination.
