Health continuance less than 20 employees

I left a job in May and requested the health continuance info be sent to me. The employer cancelled my coverage as of end of May. So I couldn't go to doctors, dentists because I had no insurance, had not money to pay out of pocket. After hounding them, they emailed me the application on June 28th.
What are my rights?
Why do I have to pay for June (retro) if I wasnt able to utilize the benefits because they cancelled it?
When are they required to send me the forms? How much time was I supposed to be given to pay, I mean I didnt know the amount until yesterday.

Does the employer have the right to cancel?
