contact your former employer - they are responsible for providing you with the COBRA paperwork and rate information.

: How can I get a COBRA premium quote? I am considering leaving my current employer and starting with another who doesn't offer health insurance. Although I'm being offered more money it may not amount to much after I pay the premium.
