In my area of the country, small groups (under 50 lives) typically have coverage that begins and ends on the first day of the month.
Larger groups can establish different rules for effective and termination dates. It is possible for the master contract to state that coverage begins on the first of the month and terminates on the last day of employment. Depending on how the contract is written, premium may be refunded on a pro-rated basis, or not at all, as it is assumed that it averages out over the term of the contract.
I have not found anything in the COBRA regulations that stipulates coverage must continue to the beginning of the next billing cycle after the qualifying event.
