If it's in the plan rules. That is unusual however. How do the regular participants pay, this is continuing coverage.

: The insurance company has demanded a lump sum payment for the entire period (several thousand dollars). Can they do this? The company pays monthly and the notice listed a monthly cost without stating that all the months had to be purchased in advance.
