If coverage ended the last day of employment, there was a qualifying event. Did they have a COBRA plan in place? Did they id what forms were missing and if so, did you turn those in?

: My former employer cashed several of my Cobra payments for dental/medical coverage but failed to inform me that the necessary paperwork had not been completed. They only informed me that the date had passed for the paperwork to be turned in when I contacted them because I was turned down for coverage at an optometrist's appointment. My former employer now says my coverage ended the last day of my employment. To complicate matters, during the lapsed period the dental insurance company that was to cover me thought I was still covered (as did I)and pre-approved several treatments which have already happened. Am I responsible for the full payments now? I never would have had the treatments if they hadn't been pre-approved.
