Sounds like a paperwork snafu. Make things really simple and if the admin is local, you might take your notices and make a trip down there. Don't leave their office until you've located the problem, assuming the person that handles that is in.

: I'm having to contact the company that administers our COBRA insurance to get three out of the four of our family members reinstated on to our prescription coverage every month despite the fact that the premiums have already been paid several months ahead of time. What is also confusing is that the only person that remains on the plan is the one person in our family that never uses the insurance. The three being dropped have all used the prescription coverage - two of us quite extensively.
