Send them a certified letter asking for the enrollment paperwork. The communication between Safeway and the third party administrator should not be your problem. They have deadlines for mailing you the enrollment forms. Especially if there was no gross misconduct.

: The health plan sent an email to SAFEWAY asking why I was terminated, but SAFEWAY has not answered. The health plan told me that without a response from SAFEWAY, I cannot be approved for COBRA; gross misconduct disqualifies one. The settlement by which I left SAFEWAY forbids it to tell prospective employers why I was terminated--but it does not forbid SAFEWAY to tell those asking for other reasons; for example, a health plan needing to know for purposes of COBRA eligibility. The health plan rep on the phone said today she would try today to get SAFEWAY to answer and get back to me today or tomorrow at latest. I was not terminated for gross misconduct, but SAFEWAY so far is not answering. What should I do?
