If the company is over 20 full time employees total, then you should have been noticed unless they area church or non profit, which it does not sound like. Also it sounds like you were not terminated for gross negligence.

The company may be liable for a penalty of $110 per day until they corret this error.

Is the company saying you got a notification for COBRA?

: I was on vacation last august in Central America and I picked up a devastating illness. Since that time I have been sending my portion of my premiums for my health insurance to my employer because I was still an employee of the company. On June 24th I received a letter stating that My employment had been terminated effective May 12th because of my inability to return to work. with this letter were three personal checks made out by myself that I had been sending them for my portion of my health insurance premiums
