If you are comfortable asking your employer about it, if they already know you will be leaving, then the easiest thing to do is just ask someone in HR or benefits. It will depend upon how big the company you work for is, or what state you are in could determine whether a mini-cobra law might apply if not big enough for federal cobra. If you are not comfortable talking to the employer about it yet, then perhaps you can call the insurance company and see if they can tell you what your continuation options would be.
