The employer has to offer you the exact same coverage you had as an employee or whatever the current employees have now(by that I mean the employer coincidentally could have been switching carriers or premiums right when you left the company so in that case your coverage or premiums might be different)

If you had vision or dental coverage on your company's policy when you worked there they HAVE to offer it to you for COBRA coverage. COBRA coverage is not a different policy. It is the same coverage (or offer of)that you had with the company and it ALL should have been listed and spelled out on the COBRA notification forms so you know exactly how much to pay and when.

I would contact your state DOL if the correct information was not given to you. More than likely you will not get another policy or coverage handbooks because you should have received those as an employee, however all coverage options and premiums should be listed.
