Your have to contact your health plan adminstrator as soon as possible. As long as you are still within the enrollment period, you should be able to apply.

Qualified beneficiaries have a 60-day period to elect whether to continue coverage. This period is measured from the date the notice to elect COBRA coverage is sent to you.

They are only obligated to either hand deliver or regular mail this COBRA notice.

If you are healthy, COBRA may not be the cheapest way to go. Get
your own health insurance with a lower premium.

: I am curious to know if I still qualify for coverage continuation because I have misplaced my forms that my employer had sent me. Should I contact them to re-send them or do you have this information on file?
