This is why I contstnatyl say send everything ceritfed mail. If you did you would have the green certifed receipt to show them. If you do not then see if the check cleared your bank. If it clerared send a copy by US Express mail return receipt requeswted to the insurance company. If the checl has not cleared then send another one by US Express Mail return receipt requested.
If the insurance company claims it is too late then contact an ERISA lawyer and sue them.
