Send a letter ceritfed mail return receipt requested to your former employer's human resource (personnel) office and ask for COBRA notice and your health plan's summary plan description. In clude your current address even if you are positive they have it.
If you do not get the informtion in 30 days contact my office so you can sue to get it and the penalties of up to $110/day for their failure.
