: Hello, we were given notification and papers that if we wanted to continue coverage we would need to fill out and sign papers. we asked our employer about them and some how the papers to fill out were miss placed and can't be found. I have now been told to find them at your web site, down load them, and fill them out and send in. However, I'm having a hard time doing so. Please help me track down the paper work that needs to be filled out to sign and get back to cobra to start our health insurance coverage again. I have a 14 mo. old baby and would like to get this resolved as soon as possible. Thank you, sincerely Greg and Janel Peddy
