* I think Kaiser shares some responsibility here to provide you with the Cobra paperwork. I would put the details into a letter to your former employer and Kaiser and request a review. The employer or the plan administrator on his behalf is required to make your rights known to you. If you do not get a prompt response I would contact your state's dept of insurance and file a complaint and if that does not produce the desired response, I would seek a lawyer to represent you.
