The problem is you really don't know who dropped the ball. Did you send your payment within 45 days after sending in your enrollment? Did they cash the check?
Remember once it does get "fixed" you would owe back premiums from last spring (since COBRA insurance is seemless) and those could be substantial.

: I was laid off last Spring and wished to continue Kaiser coverage under COBRA. It was not until June that my old employer finally got the forms to me, which I filled out and returned that same day.
: Since then, I have yet to receive any billing and Kaiser says they never received any paperwork from my old employer. Employer claims they submitted paperwork and everything should be OK. Now my Kaiser coverage is completely cancelled and I can't find out how to get it fixed.
