Subsequent to my posting the question, I found the Cobra Code that addresses this. Accordingly, a dependent child can legally elect to continue under Cobra alone. However, there is nothing that addresses what an insurance company can charge, but it seems to be commonly held the rate must be reasonable and tied to what they would charge for dependent rates or at least the single employee rates.
