The medical information may have been missing, but unless life and disability insurance are bundled with the plan (in other words you have no option) they do not fall under COBRA.

But follow up with your health plan administrator
and find out why you were not offered medical. Or contact the department of labor. Did you have the group health insurance when you left, or only the dental and vision?

: My employer was delayed in sending my cobra notice and premium information. When I recieved the letter my medical information was missing as well as life insurance and disability continuation information. It only listed premium for dental and vision. I contacted my previous employer and let them know that I had not recieved the correct information. I still have not recieved a corrected letter.

: My termination date was June 15th, 2003, coverage effective until June 30th.

: What do I do now?
