Contact your health plan adminstrator who is subing for her. ou may be able to walk in and get it handed to you.

Major medical insurance may be cheaper on your own if you have no or minor pre existing conditions.

: I got laid off and I need to fill out and turn in the Continuation Coverage Election Form but I do not have the paperwork. My human resource contact at my past job is out on sick leave. Can you please fax or send me the appropriate paperwork for me to fill out and get to COBRA asap so I can continue health coverage???
