Blue Cross would be the carrier and your employer might be the health plan adminstrator depending on if they self adminstered or not. When you leave, remember to notify your plan adminstrator of your loss of benefits so you can recieve COBRA.
Major medical insurance may be cheaper on your own if you have no or minor pre existing conditions.
: I am currently on maternity leave under the Family Medical Leave Act from my job in the private sector, and am covered under Empire Blue Cross. I would like to leave my job after this time is up. Is Blue Cross required to cover my under Cobra, or is this up to my employer?
