To Whom It May Concern:

I have a servious problem with my former employer. Because of there untruthfullness, how do I know if the premium amount I am paying is the correct amount. Also is there an administration fee suppose to be ticked onto the premium as well. I want to make sure I am not over paying them. How do I find out this information.
