Just last month, Health Net notified my mother that her COBRA coverage terminated after 6 months because the policy was terminated by her former employer.

My mother received NO notification from either the employer, OR the plan administrator. We called the employer who told us to call the Plan Administator who was not even aware my mother was covered under COBRA (yet Health Net had my mother down on record for COBRA and she has been paying the premiums past 6 mos.)

My mother had been paying about $450. Now, the Plan Administrator emailed us the "NEW" rates which are now based on age. For my mother's age tier, the rate is now $650 which is $200 MORE.

2 Questions:
1). Did the employer or Plan Administrator violate any COBRA/HIPAA/DOL or any other laws for not notifying my mother of the plan cancellation and offering her details and information of switching her over to another plan?
2). Is it legal for them to now change the billing rate to an AGE BASED rate that results in my mom paying $200 MORE per month???
