My position at work was gotten rid of and I was let go. The day I left, 4/9/04, I submitted a letter to the plan administrator, signed by the VP that I elected to accept COBRA as my daughter was scheduled for surgery in two months, and needed to have pre-op testing done, so to please continue my benefits, mail me the info and I would submit the payment checks. It is almost a month already, and I have received no notification from the plan administrator, although they say the info was mailed out to me. I am able and willing to pay the COBRA costs (but not able to pay for my daughter's needed scheduled surgery without it) and don't know what to do because they are delaying sending me the information - what are my rights? I have called them and they have been very nasty.
