My wife and have recently quit or jobs in Buffalo, NY and moved to Cleveland
Ohio. We were employed by two different companies, which offered the same
coverage plans. Our cheapest option was to be covered separate from one
another. Upon putting in my notice I rec'd the forms necessary to file for
COBRA Insurance from Independent Health. However, my wife has been told (in
writing) by Inlighten that they can't offer COBRA insurance because we have
moved out of state. Inlighten waited until past the 30 day limit to inform
us of this and even if she is entitled to the coverage it is too late to
file. Inlighten is a private company that employs more than 20 people and
to the best of my knowledge they are responsible for offering COBRA coverage
by Federal Law.

Please advise,

Jason Kopelwitz
