Cal-COBRA is California's state mini-COBRA program which generally pertains to employers with 2-19 employees.
Under AB 1401, California extended continuation to qualifying residents to a total of 36 months. The first 18 months in your case would be under federal COBRA as your employer has 20 or more employees. If you are on a qualifying plan, Cal-COBRA would take over the second 18 months at your option and would be administered directly by the health insurance company (as all Cal-COBRA is).
The key is that you are insured by a California-domiciled employer with a California-based, fully-insured health plan. If that is the case, you will receive a letter about 30-60 days prior to your last federal COBRA date from the health insurer which will give you the option to elect the extension under Cal-COBRA.
A note which is important to any California employee, this extension does not apply to self-funded or self-insured plans nor does it apply to a California employee of a company domiciled outside of California which insures the California employees through another state's health plan.
