You have 60 days to elect COBRA, so you might still be in your election period, but if so, it will be ending very soon. I would call your ex-employer and tell them you misplaced the paperwork and ask them about the COBRA, how much is it, when is your election period over, what coverage would you be able to get since you moved out of state...
If your election period has expired, then you would need to look into private insurance.
