Does your wife qualify for Family Medical Leave? If so, then she would need to receive her benefits throughout her 12 weeks of leave, at the same level as active employees.

If not, then yes, the company can terminate her coverage and offer cobra instead, as she is not an active employee at this time, and most insurance plans require employees to be in an active status. Under COBRA, the employer does not pay any part which is why it would be so much more expensive.

Do you have insurance through your employer? It might be cheaper to add her to your plan instead of using the COBRA.
